Your turtle could have either a Vitamin A deficiency or an infection or both. The best thing you can do is take the turtle to a vet. The vet will give the correct medication. The vet will also talk with you about keeping the habitat really clean.
This is a serious condition. Your red eared slider could become blind. Then it won't be able to find food. Turtles find food by using their eyes.
