It can both crack his shell and do internal damage, get him to a vet asap...be very careful when ever you carrying your Turtle or Tortoise. I suggest putting your Tort in a container that you are more able to have a good hold onto, and then sit and hold your Tortoise to get used to how they react when you pick them up.
