female spotted turtle with shell trouble on bottom maybe?

My turtle has a perfectly symmetric circle hole on the bottom of her shell. she a female yellow spotted turtle that is less than a year old. I keep the tank clean and treat water before hand. Some days its bigger other smaller and I've even seen it gone for a short time. is this any cause for alarm?
