Help!

I have a young horsfields tortoise, she is around 16 months, we have had her for 5. she is in a vivarium with a larger, 7 year old male horsfield, UVB light,heat mat and heat lamp. she is fed on dark leafy greens and rocket. recently, i have noticed that the bottom of her shell is arched downwards, and is slightly soft. i got her out today, and noticed a tiny hole in her shell, like the size of a needle hole, and a slightly larger one around a mm away from it, located in a part where she has been growing and is not quite hardened up yet.she is small enough to fit in the palm of my hand, whereas my male tortoise is quite a bit bigger. she is acting normally, eating and walking around, but i dont know what has caused the hole in her shell.
