Hingeback tortise wont eat

I have a hingeback tortoise approx 30 years old.He has a dry crusty area around his right ear and wont eat.Have taken him to a vet who said all is o.k. but I don hink so.He will always eat live worms and this timewont. Any ideas?

maybe try some other kinds of insects , or a variety of food ? or even get a second opinion from another vet ? I know with my own health problems the doctor is not always correct .
