Although a penis will come out from time to time, it's not usually red. You are more likely dealing with a prolapse. It would be good to see a vet about this both to find the cause and to find out what you can do about it.

Well, there's really no other way to say this, but you almost certainly a male tortoise and what you're seeing is his penis. It was quite shocking when I first saw this odd-looking red thing coming out of his tail, but after some research (and several more recurrences of Hector showing his privates) I figured out that it was his sex organ. This may not have been the answer that you were expecting, but I'm quite confident that this is what you're seeing based on your description.
