I agree with the previous poster. You probably also need to re-evaluate your husbandry. Do you have a place that the turtle can haul out of the water and completely dry off? Do you have the proper lighting? Heat and UVB?

See a vet. The best choice is to see one that specializes in tortoises and turtles.
A hole in a shell can be very serious. Once a turtle or tortoise gets an infection under the shell, it can be very hard to treat. Your pet could die without proper care.
