I have had this turtle for about five years and a few days ago, I noticed that the top of his head has sunk down so it looks like he has a dent in his head? Any idea why?

It sounds as if he is not getting the proper calcium and or sunlight enough,,,also are you feeding him the proper food for his care, pls take him for a checkup to your local vet.
